Facility
A primary function of our staff is to assist residents and families with comprehensive
discharge planning, including but not limited to having home health care arrangements and
continued out patient services as required. Assistance is also provided to residents and families
in obtaining Medicaid eligibility for those who choose to make Hamlin Place their home.
Our qualified professional staff is always available to help address the emotional, financial and social
issues of our resident and families.
